USE OF TRAINING /BEHAVIOR MODIFICATION TOOLS: With an overwhelming request from our current clientele- we do include the use of e-collars. While in the care of House of Wag, we do work on behavior training, and what is appropriate and what is not. We work to correct negative group behavior and socialization with redirection techniques, verbal commands, noise distraction, and when necessary, the use of e-collars. While we have our own supply of them- owners may also bring in their own. The collars we have and those that are provided by owners MUST have the option for us to turn OFF the shock option. We use them as an aid to correct and control excessive barking, "police" barking, impulse control, and/or overly aggressive behavior in play using the vibration mode and beep mode exclusively. Since the dogs we use them on- already use them at home, they are aware of the warning vibrations and beeps. The collars are never used for "capital punishment." They are extremely useful aids at helping to target with a beep, a vibration, or a combination a specific dog, instead of having to place a dog in isolation out of group play, set a negative vibe because we're constantly having to verbally correct the excessive barking due to the dog's play nature, over stimulation, excessive humping, etc. They respond to correction, get positive reinforcement verbally for continuing positive play. House of Wag has extensive experience with reading dog body language, training and working with multi-dog group play and continuously researching and learning different techniques to help keep our play groups fun, enriching, and safe. Not every dog needs or requires the use of the e-collar. By signing this form, you acknowledge that you understand and accept the terms and conditions set forth by this agreement.

HEALTH AND TEMPERAMENT AGREEMENT: I agree and understand that in admitting my dog to House of Wag Daycare that my dog is in good health, is current on all vaccinations and flea control and has not harmed or shown aggression or threatening behavior towards another dog and/or human.I understand that in any cageless dog environment that there is an inherent risk of injury or illness from rough play and/or fights. Understanding this, I accept full responsibility and hold House of Wag, harmless for any pet injury, death or damage.I agree that I am solely responsible for any harm caused by my dog while my dog is in the care of House of Wag.I agree not to hold House of Wag and their associates liable for any injuries to my dog while in the care of House of Wag.I understand if my dog shows repeated aggressive or menacing behavior that the dog will be moved to seclusion. If the behavior continues your dog may be asked to leave House of Wag.By signing this form, you acknowledge that you understand and accept the terms and conditions set forth by this agreement.

The owners, staff, partners and volunteers, will not be held liable, financially or otherwise, for injuries to my dog, me, or property of mine while my dog is participating in services provided by House of Wag, LLC. I hereby release House of Wag, LLC of any liability of any kind arising from my dog’s participation in any and all services provided by House of Wag, LLC.

Any problems with my dog; behavioral, medical or otherwise, will be treated as deemed best by the staff of House of Wag, LLC in their sole discretion and in they view as the best interest of my dog. I understand that I assume full financial responsibility and all liability for any and all expenses involved in regards to the behavior and health of my dog while participating in any and all services offered by House of Wag, LLC.

There are risks and benefits associated with group socialization of dogs. I agree that the benefits outweigh the risks and that I accept the risk. I desire a socialized environment for my dog while attending services provided by House of Wag, LLC, and while in their care. I understand that while the socialization and play is closely and carefully monitored by House of Wag, LLC staff to prevent injury, it is still possible that during the course of normal play, my dog may receive minor nicks, cuts, scratches and puncture marks from other dogs. Any injuries to my dog will be properly cared for by the staff of House of Wag, LLC, and I will be informed of the injury upon pick-up

By allowing my dog to participate in services offered by House of Wag, LLC, I agree to allow House of Wag, LLC to take photographs and/or use images of my dog for print, social media, advertisement, and/or promotion at their sole discretion.

The owner of the dog(s) is solely responsible financially or otherwise, for any harm or damage caused to another dog by their dog, while participating in any services provided by House of Wag, LLC.

If a dog is not picked up on time or on the date specified in a separate agreement for boarding, I hereby authorize House of Wag, LLC to take whatever action is deemed necessary for the continuing care of my dog. I will pay House of Wag, LLC the cost of any such continuing care upon demand by House of Wag, LLC. I understand that if I do not pick up my dog, or contact, House of Wag, LLC within 72 hours of not picking up my dog on the specific date and time I had signed and agreed to, they will proceed according to the guidelines provided by the City of Fort Worth’s abandoned animal rules. I also understand that I will be fully responsible for all attorney’s fees and associated costs, if I abandon my dog.
required for all House of Wag, LLC participants receiving any of the services House of Wag, LLC offers. First and foremost, the safety and well-being of your pet(s) is of the highest importance. Insuring that your pet remains safe and well cared for is our first responsibility and as such we take it very seriously. We do our best to have all pet parents screen for pre-existing health conditions, but some factors may be beyond our control. In the event that a medical emergency arises while a pet is at our facility or participating in a service that we provide, it is imperative that we are immediately able to get them medical treatment at the closest available vet facility. We will call ahead to the veterinarian offices in the closest proximity geographically to our location, to insure that they can handle the emergency present. Your pet will be rushed to the closest facility and the owner will be contacted. We notify the owner after we have secured a medical treatment center so the animal can avoid any delays in medical treatment that may be caused by emotion on the part of the owner. Our goal is to get the pet medical attention as quickly as humanly possible, and any distractions or delays may interfere with that process. For that reason, it is a requirement to have our pet parents sign this form.     In the event of a medical emergency, that House of Wag, LLC, at its sole discretion, deems to need the immediate attention of a licensed veterinarian, I authorize House of Wag, LLC to seek medical attention at the closet available veterinarian facility. I further agree that I am financially responsible for any medical treatment my pet(s) receive as a result of a medical emergency while attending any of the services provided by House of Wag, LLC.
